Electronic paperwork

You can fill out your medical form at home or in your cab! Just give us your phone and/or email when you schedule online. Alternatively, when you arrive at our clinic you will be greeted by our staff and given the DOT iPad to get started on filling out your medical form. This will ask questions about your medical history as well as any medications you are on.

Physical Exam

After completing the DOT form we will begin the exam. The exam consists of an evaluation of the major body systems such as the heart, lungs, ears, eyes, etc.

Urinalysis

Part of the physical exam includes a urinalysis. A urinalysis checks the urine for the presence of sugar, protein and blood. This is NOT a drug test and does NOT check for the presence of drugs.

Results

After completing the exam, Dr. Huebner will review the results with you and give you the certification, if qualified. If further testing is needed or other exam findings need reviewed, they will be discussed here.

The FMCSA has implemented Guidelines and Regulations that drivers must adhere to in order to be Certified. Here are some common regulations and helpful tips:

  • A diagnosis of epilepsy disqualifies you from receiving a medical certificate
  • Marijuana, even for medical purposes, disqualifies you from receiving a medical certificate
  • Your vision needs to be 20/40 or better and you must be able to hear a whisper from 5 feet
  • If you use a CPAP for Sleep Apnea, you need to bring your compliance log. If you need help with that, contact the company that gave you your CPAP
  • Caffeine and Nicotine may increase your blood pressure and reduce the time length of your certificate (DOT medical card). Please don’t drink caffeine or use tobacco for 2 hours before your exam
